Our process for Hospital Discharge admissions are completed via an appointment/rapid-assessment with the professionals involved in a patients care and the patient themselves whilst in Hospital. If a package of care is agreed, the Coordinators will complete a care plan and assign the patient their very own care team within a couple of days and liaise with Hospital Discharge Coordinators to get the client home to a safe, holistic care package.
